How It Works
Becoming an Immanuel at Home member is easy. Contact us today to learn more.
Interested new members may request a sign-up appointment for either Club Immanuel or Immanuel HomeCare in person, online, or by calling
Assess Your Needs
Once a Member, you will receive a call from your Concierge Service Coordinator (CSC), who will schedule an assessment in the coming days.
Your assessment for your Plan of Care will be done during your sign-up appointment with the Immanuel HomeCare staff.
Orientation & Support
Your CSC (Concierge Service Coordinator) will provide orientation and training, as well as scheduling ongoing support.
Your Personal Care Attendant (PCA), Certified Nursing Assistant (CNA), or Companion will arrive at your home to perform HomeCare services at the agreed-upon ongoing schedule.
